چکیده

A 2-year-old girl with no significant family history presented with motor developmental delay and strabismus. MRI revealed unilateral basal ganglia and brainstem lesions (figure 1). Eighteen months later, she developed acute onset right arm weakness, leading to a diagnosis of multiphasic disseminated encephalomyelitis. Treatment with steroids and mycophenolate produced no symptomatic or imaging improvement. Her condition progressed, with ataxia, multifocal dystonia, spasticity, and loss of ambulation at age 6 years. Serial imaging showed evolution of bilateral basal ganglia changes, compatible with Leigh syndrome (figure 2). A heteroplasmic mutation, m.12706T.C in the MTND5 gene, was identified in muscle.

Teaching neuroimages: MRI appearances of Lhermitte-Duclos disease.

A 48-year-old woman was admitted with a 3-year history of intermittent dizziness and unstable gait. Cranial MRI demonstrated a laminated lesion of T2 hyperintensity and T1 hypointensity involving the left cerebellar hemisphere, which appeared enlarged (figure, A–C). Secondary hydrocephalus and Chiari I malformation were observed. Because the striated appearance on MRI was characteristic of Lher...

Teaching neuroimages: neuroradiologic findings in pontine and extrapontine myelinolysis: clue for the pathogenesis?

A 58-year-old woman presented with subacute onset of tetraparesis, anarthria, and dysphagia after rapid correction of hyponatremia caused by repeated vomiting. Brain MRI showed pontine and extrapontine lesions typical for the osmotic demyelination syndrome (figure, A and B). The pontine lesion showed nonhomogeneous contrast enhancement (figure, C), which diminished after 15 days (figure, D). Ba...

Neuroradiologic findings in children with mitochondrial disorders.

PURPOSE We report the neuroradiologic findings in 25 children with various mitochondrial diseases. METHODS Twenty-two children with a mitochondrial disorder had MR imaging of the brain and three children had CT studies. In all cases, the diagnosis was based on examination of muscle morphology, analysis of oxygen consumption and respiratory chain enzyme activity in isolated muscle mitochondria...
